• Community Health Worker certification or 2000+ hours of CHW work
• 1-3 years healthcare experience
• 2-5 years community work experience

Responsibilities

• Assist patients with health navigation
• Conduct outreach to patients
• Engage with patients for care coordination
• Document patient interactions
• Participate in community events
• Provide health education
• Support goal setting
• Conduct social needs assessments
