Requirements

• Bilingual (English/Spanish) preferred
• Experience with Social Determinants of Health preferred
• Previous canvassing or community organizing experience preferred

Responsibilities

• Meet members at their homes
• Promote ECM and CalAIM services
• Facilitate Connect for Life activities
• Build relationships with members
• Encourage community participation
• Liaison between members and health plans
• Partner with local organizations
