Overview

Role involves delivering remote comprehensive clinical care via text and video.

Ideal candidate should have 3+ years of experience and be board-certified in Family or Internal Medicine.

Must be licensed in CT, IMLC preferred

Requirements

• Active CT medical license required
• 3+ years post-residency experience
• Board-Certified in Family or Internal Medicine required

Responsibilities

• Manage a dedicated panel of patients
• Educate patients on treatments and care plans
• Manage patient follow-ups for chronic care
• Prescribe and refill medication
• Drive high-quality care focused on patient outcomes
• Collaborate with Clinical Team Members
• Elevate remote care with meaningful patient encounters
